Step 2Ìýâ–¡Ìý

Submit copies of your Verification of Disability

(Email completed copies toÌýMCACCESS@VCCCD.EDU)

  • Disability verification is generally a letter or document provided by a qualified professional that identifies the nature of the condition and the resulting functional limitations. IEPs and 504 plans are also considered disability verification forms.

  • Students with an IEP or 504 plans from high school: PleaseÌýsubmitÌýa complete (include all pages) IEP or 504 plan, and a copy of aÌýrecent psycho-educational assessment report, if available.

  • If you don’t already have a verification of disability, we have aÌýDisability of Verification FormÌýavailable to assist you with obtainingÌýdisability documentation from a healthcare professional.

Once steps 1 and 2Ìýhave been completed, we willÌýcallÌýyou toÌýset up an intake appointment
